Organization Background
Kopano Ke Matla, meaning ‘unity is the power’ was established on the 16 October 2003. It is situated in Mabopane North of Tshwane, doing care and support to patients. members of this organization were encouraged after realizing the challenges facing the community, number of these challenges are:-
· Increased number of hospitalizations due to HIV/AIDS related illness.
· Lack of care and support to terminally patient at home.
· The stigma related with HIV/AIDS infected and affected and families.
· The needs to support and protect the rights of children who lost their parents due to HIV/AIDS related illness.
The organization in partnership with other NGO’s, Department of Health, Social Development assisting in pulling resources together for more effective and efficient response. This has and will also reduce the number of high cost effective admission by hospital. The health facilities e.g. clinics and hospitals refers the patient to the organization and the organization also refers to the hospitals for treatment and medications. This provision of home care reduces the burden carried by few public hospitals surrounding Mabopane Township.
Another service that the organization is providing is the VCT Mentoring Programme. This programme deals with mentoring, supervising and counseling of HIV/AIDS counselors in seven sites around Mabopane and Winterveldt.

1. Definition of the Project
Kopano ke matla is a Community Home Based Care project, providing comprehensive services, including health and social services. Care-givers provide these services at home to restore and maintain client’s comfort and care towards dignified death.
Home-carers services are classified into:-
· Prevention strategies to clients
· Health promotion
· Therapy
· Moral support
· Palliative care categories.
· Counseling
The community can access the free service from a walking distance. Community Based Care responds to the needy people and encourages participation and responsibility.
1. Background/Statement of the Problem to be Addressed by the Project
a) Proper care and support for people affected and affected by HIV/AIDS
b) Less accessibility of health facilities in the community
c) Alleviate poverty amongst the community that we serve.
d) Long waiting by patients at local health facilities for minor ailments
e) Long waiting period for grants application, where other clients die before receiving a grant.
f) To cover two areas that are not budgeted in the current funding i.e. Boekenhout and Mabopane
Interventions
Kopano Ke Matla is under Mabopane Health Forum called Mabopane Health Area, in partnership with FBO, Government Departments, Lifeline and private sectors, hospitals and clinics.
1. The purpose/Objectives of the Project
· To implement community Home Based Care projects to be sustainable.
· Mobilization strategy to reduce risk factors.
· Advocacy
· Treatment and care to infected and affected.
· To engage food gardening activities, art and urban greening.
· To create community awareness on HIV/AIDS to reduce stigma related to the epidemic.
· To establish poverty alleviation program by encouraging community income generating activities e.g. food gardening.
· The organization secondary objectives will be to work in partnership with other organizations e.g. hospice, clinics and hospitals.
1. Results of the Project
Kopano ke Matla provide care and support to 300 people including those with HIV/AIDS.
The organization offers palliative care to very sick patients and encourages participation of community care-givers to reduce poverty. Care-givers promote treatment compliance to reduce the spread of diseases. Counseling to patients reduces stigma related to HIV/AIDS, and bereavement counseling to children who lost both parents.
